About Community Pathways WA

Supporting Independence and Well-being

Community Pathways WA is dedicated to helping individuals build stable, independent lives through supportive housing and employment services. We believe that everyone deserves safe housing, meaningful work, and access to the support they need to thrive in their community.

Our team provides person-centered case management, housing stability support, and employment assistance tailored to each individual’s goals, strengths, and challenges. We work closely with community partners, healthcare providers, landlords, and service agencies to create coordinated support systems that promote long-term success.

We serve individuals experiencing homelessness, housing instability, and barriers to independence, including disabilities and behavioral health challenges. By meeting people where they are and walking alongside them through each step of their journey, we help create real pathways to stability, self-sufficiency, and hope.

Services

Supportive Housing

Community Pathways WA supports individuals experiencing homelessness or housing instability, including those facing challenges such as disabilities or substance use.

Quality housing
Our supportive housing units are safe, well-maintained, and similar to other homes in the community.

 

Integrated support
Clients receive housing and services in inclusive community settings that promote independence and belonging.

Stable, permanent homes
Tenants have standard lease rights and can remain in their homes as long as lease terms are met.

 

Affordable living
Housing costs are kept within affordable limits to support long-term stability.

Coordinated care
Our housing and support teams work closely together to prevent evictions and ensure clients receive the help they need.

Our Supported Employment Services help individuals who want to work find and keep meaningful jobs in the community. We provide personalized support based on each person’s goals, strengths, and needs.

Services may include:

  • Exploring career interests and job options

  • Building relationships with local employers

  • Help with applications, interviews, and hiring

  • On-the-job coaching and skill development

  • Support with workplace communication and relationships

  • Guidance on self-advocacy and workplace accommodations

Our goal is to help each individual succeed in long-term, integrated community employment.

Supported Employment

Community Support Services

Community support

Community Support Services (CSS) help individuals prepare for housing, move into a home, and stay housed long term. We also work behind the scenes to coordinate care, search for housing, and communicate with landlords to support each client’s success.

Pre-Tenancy Support Includes:
  • Assessing individual needs, preferences, and housing goals

  • Helping clients identify the type of housing that works best for them

  • Support with budgeting and understanding housing costs

  • Assistance with applications for income, benefits, and social services

  • Help finding housing that meets medical and personal needs

  • Creating a personalized community integration plan focused on long-term stability

Tenancy-Sustaining services include:
  • Personalized service planning and ongoing care coordination

  • Connections to healthcare, mental health, substance use treatment, and community resources

  • Help applying for benefits and financial supports

  • Support with independent living skills such as budgeting and household management

  • Assistance communicating with landlords and requesting accommodations

  • Regular housing stability and crisis plan check-ins

  • Tenant education to support lease compliance and successful housing

How Do I Get Started?

Click here to fill out a short form. We will contact you within 1-3 business days to schedule an assessment for services.

The Assessment

No need to waste your money on bus fare or gasoline. We will come to you to provide a Housing and/ or Employment Assessment. This process takes about 30 minutes. 

The assessment goes over your housing and/ or employment history and identifies risk factors and barriers that will qualify you for our program.

FCS Approval

Once your assessment has been submitted, we will receive an approval or denial within 5 business days. If it is denied, we will make any corrections/ provide further paperwork needed and resubmit. If approved, you will be assigned a case manager right away.

Begin Receiving Housing and/or Employment Support Services

Start identifying goals and the steps needed to help you reach them. Create an individual service plan to map out the process of achieving stability and independence!
