Do I Qualify for FCS Supportive Services?
If you receive Medicaid (state medical insurance) and are currently enrolled in Washington Apple Health and have risk factors, such as:

Chronic Homelessness: Defined as being homeless for 12 months or more, or on 4 separate occasions in the last 3 years.

Behavioral Health: A diagnosed mental health condition or substance use disorder.

Complex Health Needs: Significant mental illness, chronic physical conditions, or needing assistance with 3 or more Activities of Daily Living (ADLs).

Institutional Transition: Recently in a nursing facility, hospital, or prison.

Housing/Benefits Programs: Enrollment in the Housing and Essential Needs (HEN) or Aged, Blind, Disabled (ABD) program.
